What do you have to do?

All you have to do is go online and download the form to submit, following the directions and you will be set. You will need your receipt, the small upc label from the intake box and the large upc label from the intake box. Then complete the form and mail it in. Reminder, all purchases must be between August 1 and September 30th. All rebate forms need to be submitted by October 30, 2014 to qualify.
